Product Update: Marking Diagnostic Reports as Entered in Error

Overview​

This release introduces a new Entered in Error workflow for diagnostic reports in OmegaAI. Previously, deleting a report permanently removed the report and its associated data from the system. With this update, users can mark Final and Preliminary reports as Entered in Error, ensuring that report data, audit history, author information, timestamps, and linked observations remain available while clearly indicating that the report is no longer clinically active. This change supports clinical record retention requirements and provides greater visibility into report history.

What's New​

1. New Entered in Error Workflow​

What changed:​

The previous Delete action has been replaced with Delete (Entered in Error). When selected, the report status changes to Entered in Error instead of being permanently removed from the system.

Benefit:​

Allows reports to be withdrawn while preserving all associated report information and audit records.

2. Updated Report Status Indicators​

What changed:​

Several visual updates have been made within the Document Viewer, including:

  • Delete button renamed to Delete (Entered in Error)
  • Trash icon replaced with a cross icon
  • Signed indicator with a green tick icon for Final Reports
  • Entered in Error status displayed in the Patient Chart
  • Updated icon colours for current and prior studies

Provides clearer visibility into report status and available report actions.

3. User Access Control for Entered in Error Reports​

What changed:​

A new User Access Control permission called View Entered in Error Reports has been added under the Document Viewer section. Reports marked as Entered in Error remain visible only to users assigned this permission.

Benefit:​

Provides organizations with control over who can view reports that have been withdrawn from clinical use.

Important Notes​

  • Reports marked as Entered in Error remain in the system and retain all associated report data, including audit trail information, author details, timestamps, and linked observations.
  • Reports marked as Entered in Error are only visible to users with the View Entered in Error Reports permission.
  • This permission is enabled by default for Default Admin Role, Radiologists, Technologists, and Admin roles.
  • Final Reports and Preliminary Reports can both be marked as Entered in Error.
  • Reversing Entered in Error status on a Final Report places the report into the Amended workflow.
  • If a Final Report already has an amendment, it cannot be edited after reversal.
  • Reversing Entered in Error status on a Preliminary Report restores editability, allowing the report to be finalized and signed.
  • Updated tooltips are displayed for report action icons.
